Here's how to receive your rebate:

1. Get a contact lens fitting and eye exam! Purchase has to be made within 90 days of exam.

2. Purchase the required number of boxes. Provide a valid email address on your order form. A mail-in rebate form should be included in your package or may be mailed to you separately.

3. Complete the mail in certificate. Attach your sales receipt (available to print from your account once your order has shipped) and a copy of your valid eye exam and contact lens fitting receipt.

4. Mail in the original package UPC codes, your sales receipt indicating total purchase quantity, a copy of your original eye exam and a contact lens fitting receipt and the mail in certificate to:
